Brady Perkins dies after injuries sustained at Butler Merchandising Solutions

St. Louis (KSDK) - A worker at Butler Merchandising Solutions died late Tuesday night due to an industrial accident. 

Brady Perkins, 47, was working at a clam shell die machine when he reached in and the machine closed, causing trauma to his head and torso.

The St. Louis Fire Department received a call around 9:18 p.m. to head to the scene in the 2200 block of Delmar.

The Fire Department responded, and Perkins was pronounced dead at the scene.

OSHA will be notified. The incident is under investigation.
